This appears to be due to a defective z-wave me module. I swapped the module and the device is now included.

Also :

wget -q -O - razberry.z-wave.me/install/v2.0.1-rc29 | sudo bash

Tried to reset controller to factory default.

Typically, is such a problem with the controller software or with the device?

How can I help fix this?

I tried with latest

wget -q -O - razberry.z-wave.me/install/v2.0.1-rc25 | sudo bash

No change.
