Nodes failing or even disappearing for no reason

I'm still having issues with randomly failing devices after upgrading to 3.1.4. Nodes will stop working or disappear completely from the network.
When the node stops working, the software just reports "node appears to be dead". In most cases, doing a Call for Nif and Force Interview will restore functionality. In two cases (both devices being a Fibaro 221 dual relay switch) I had to power cycle the devices before the interview would complete; I suspect hardware/firmware issues, though I haven't had to do that even once in all the years I was still using a Vera Z-Wave controller.
In other cases, the node silently disappeared from the network. In this case, when I put the controller in inclusion mode and hit the device inclusion button, the device reappears with the old device ID, and works as before. It does not work however when the node was included securely; apparently the keys are lost and my only recourse is to mark the node as failed and do a Replace.
